Scheduling your first appointment...

When you call (208-376-0567) our office, our friendly staff will help you coordinate your
initial visit. They will also be glad to answer any questions you may have regarding your
first visit and help ensure you know what to expect.
To help your first visit move as smoothly as possible, please complete and print the HIPPA Agreement, the Medical History, the New Patient Information and Dental History, and the Financial Agreement Forms prior to your visit. If you so desire you may fax (208-376-0661) the completed forms to our office
prior to your scheduled appointment. If you choose wait or need help to fill out the forms at
your first visit, please arrive about 25 minutes prior to your scheduled appointment to provide adequate time to complete
the documents.
What should I expect during my first appointment?
Your first visit will include an initial discussion with the doctor; providing you an
opportunity to present and discuss your dental needs and concerns.Immediately following the short discussion Dr. Gurney will performa comprehensive oral examination. This examination will include: an head and neck cancer screening, a TMJ evaluation, a dental, a periodontal, an occlusal evaluation, an
esthetic evaluation, photos, and any needed x-rays/radiographs. Depending on the extent of the findings the initial findings will be presented and discussed. In order to help minimize confusion the possible treatment options specific for your situation will be presented at a subsequent consultation visit. Complex dental treatment often requires additional information to engineer
a predictable definitive treatment plan. Thus, a definitive treatment plan may not be
presented until all needed information and treatment coordination is completed. Any additional information deemed essential to properly develop the treatment plan that satisfies your needs and desires will be determined during the treatment avenue presentation consultation.
Our
goal is to engineer a treatment plan that is predicable and successful long term, which
will meet your expectations.
Are there fees associated with the initial examination?
Examination fees will be associated with this initial visit. If you choose not to proceed with treatment in our office the initial
examination fees are non-refundable.
